Job summary

Trinity Healthcare Talent Services is seeking a skilled Physical Therapist to join a small Continuing Care Retirement Community (CCRC) with a 58-bed Skilled Nursing Facility. The PT will provide physical therapy to residents in short-term, long-term, and outpatient settings for Assisted Living and Independent Living residents.

The role requires current licensure, at least 1 year in SNF or similar settings preferred, and knowledge of Medicare guidelines.

Qualifications

• Current state license to practice as a Physical Therapist.
• Minimum of 1 year of experience in a skilled nursing facility or similar setting preferred.
• Knowledge of Medicare guidelines and reimbursement for physical therapy services.
• Experience with electronic medical records preferred.

Responsibilities

• Conduct initial evaluations and develop individualized treatment plans for residents based on their needs and goals.
• Administer prescribed physical therapy treatments, including therapeutic exercises, manual therapy, and modalities.
• Monitor and document residents' progress, adjusting treatment plans as needed.
• Communicate regularly with the interdisciplinary team to coordinate care and ensure the best outcomes for residents.
• Educate residents and their families on proper techniques and exercises to improve mobility and function.
• Participate in care conferences and discharge planning for residents.
• Adhere to all state and federal regulations and professional standards in the delivery of physical therapy services.
