Job summary

Trinity Healthcare Talent Services, LLC is seeking a skilled Physical Therapist to join their team at a small Continuing Care Retirement Community (CCRC) located in Carlisle, Cumberland County. The P-Physical Therapist will be responsible for providing tailored physical therapy services to residents in both short-term and long-term care settings, including outpatient services for Assisted Living and Independent Living residents.

The ideal candidate will hold a valid state license, possess strong communication skills, and have a strong background in skilled nursing facilities. A minimum of 1 year of experience is preferred, and knowledge of Medicare guidelines is essential. This position offers a competitive hourly wage ranging from $41.00 to $50.00.

Position: Physical Therapist

Salary Range: $41.00 to $50.00 per hour

Hours: 35 hours per week, Monday-Friday with weekend rotation

Facility: Small Continuing Care Retirement Community (CCRC) with a 58-bed Skilled Nursing Facility (SNF)

Job Summary:

We are seeking a skilled and compassionate Physical Therapist (P-Physical Therapist) to join our team at our small CCRC with a 58-bed SNF. The P-Physical Therapist will provide physical therapy services to our residents in both short term and long term care settings, as well as outpatient services for our Assisted Living and Independent Living residents.

Key Responsibilities:

- Conduct initial evaluations and develop individualized treatment plans for residents based on their specific needs and goals

- Administer prescribed physical therapy treatments, including therapeutic exercises, manual therapy, and modalities

- Monitor and document residents' progress, adjusting treatment plans as needed

- Communicate regularly with the interdisciplinary team to coordinate care and ensure the best outcomes for residents

- Educate residents and their families on proper techniques and exercises to improve their mobility and function

- Participate in care conferences and discharge planning for residents

- Adhere to all state and federal regulations and professional standards in the delivery of physical therapy services

Qualifications:
