Trinity Healthcare Talent Services, LLC is seeking a skilled Physical Therapist to join their team at a small Continuing Care Retirement Community (CCRC) located in Carlisle, Cumberland County. The P-Physical Therapist will be responsible for providing tailored physical therapy services to residents in both short-term and long-term care settings, including outpatient services for Assisted Living and Independent Living residents.

The ideal candidate will hold a valid state license, possess strong communication skills, and have a strong background in skilled nursing facilities. A minimum of 1 year of experience is preferred, and knowledge of Medicare guidelines is essential. This position offers a competitive hourly wage ranging from $41.00 to $50.00.

Qualifications

Responsibilities

• Conduct initial evaluations and develop personalized treatment plans based on specific needs.
• Administer prescribed physical therapy treatments including exercises and manual therapy.
• Monitor and document residents' progress, adjusting treatment plans as needed.
• Educate residents and families on proper techniques to improve mobility.
