Job summary

The Center for Physical Rehabilitation seeks a motivated PRN Physical Therapist to join outpatient orthopedic clinics in Grand Rapids. You will assess patients, develop treatment plans, and deliver direct care within established protocols.

You will also document progress and coordinate with caregivers, physicians, and insurers to optimize outcomes. Join our friendly, fast-paced clinics with extended hours, working Monday–Friday, 7 a.m.–7 p.m.

Qualifications

• Graduate from CAPTE-accredited Physical Therapy program.
• Current state MI license and CPR certification.
• Ability to work autonomously in an outpatient setting.
• Basic PC skills and EMR familiarity.

Responsibilities

• Assess patients to develop and initiate treatment plans based on evaluations.
• Review physician referrals and patient records to determine needed therapy.
• Provide direct patient care per established protocols and guidelines.
• Document findings, progress, and instructions to patients and caregivers.
• Supervise assistants and rehabilitation technicians in care delivery.
• Develop a written plan of care and goals based on evaluations.
• Implement the physical therapy program and essential treatments.
• Communicate with patients, caregivers, physicians, and insurance providers.
• Ensure timely documentation including daily notes and reports.

Our state-of-the-art facilities are located throughout greater Grand Rapids with extended hours. Independent with local ownership since 1994, we have the freedom to work with the most qualified healthcare professionals in West Michigan. We're looking for a motivated and enthusiastic PRN Physical Therapists to join our fun, energetic outpatient orthopedic clinics in the Grand Rapids area.

Our clinics operate Monday-Friday, 7 a.m.-7 p.m.

• Assess patients to develop and initiate treatment plans based on the evaluation results
• Review the physician's referral (prescription) and the patient's condition and medical records to determine the physical therapy treatment required
• Provide direct patient care by established protocols and patient care guidelines
• Document findings, progress, and instructions to patients and caregivers
• Responsible for patient care performed by assistants and rehabilitation technicians
• Complete a written plan of care, develop goals based on evaluation findings, and develop a comprehensive program to attain goals
• Implement physical therapy treatment program and provide essential physical therapy treatments
• Communicate effectively with patients/caregivers and professional colleagues, including physicians and insurance companies
• Ensure timely documentation, including daily notes, weekly workers' compensation reports, progress reports, initial evaluations, discharge summaries, and incident reports

• Graduate degree from CAPTE-accredited Physical Therapy program.
• Current state of MI license and CPR certification
• Ability to work efficiently/effectively in an autonomous environment
• Basic PC skills and knowledge, including but not limited to Electronic Medical Record (EMR) system, database software, and MS Office,
