Job summary

The Center for Physical Rehabilitation in Grand Rapids is seeking motivated PRN Physical Therapists for our outpatient orthopedic clinics. You will assess patients, plan individualized care, and deliver treatment according to established guidelines.

Clinic hours run Monday–Friday, 7 a.m.–7 p.m., offering a flexible PRN schedule in a supportive, team-based environment. Ideal candidates hold a CAPTE-accredited PT degree, Michigan license, CPR certification, and are comfortable working autonomously

Qualifications

• Graduate degree from CAPTE-accredited Physical Therapy program.
• Current MI license and CPR certification.
• Autonomous work in a clinical setting.
• Proficient PC skills including EMR, database software and MS Office.

Responsibilities

• Assess patients and develop treatment plans based on evaluations.
• Review referrals and patient records to determine required PT treatment.
• Provide direct patient care per established protocols and guidelines.
• Document findings, progress, and patient care instructions.
• Supervise assistants and rehabilitation technicians as needed.
• Develop written plan of care and goals based on evaluations.

We’re looking for a motivated and enthusiasticPRN Physical Therapiststo join our fun, energetic outpatient orthopedic clinics in the Grand Rapids area.

Our clinics operate Monday-Friday, 7 a.m.-7 p.m.

• Assess patients to develop and initiate treatment plans based on the evaluation results
• Review the physician's referral (prescription) and the patient's condition and medical records to determine the physical therapy treatment required
• Provide direct patient care by established protocols and patient care guidelines
• Document findings, progress, and instructions to patients and caregivers
• Responsible for patient care performed by assistants and rehabilitation technicians
• Complete a written plan of care, develop goals based on evaluation findings, and develop a comprehensive program to attain goals
• Implement physical therapy treatment program and provide essential physical therapy treatments
• Communicate effectively with patients/caregivers and professional colleagues, including physicians and insurance companies
• Ensure timely documentation, including daily notes, weekly workers' compensation reports, progress reports, initial evaluations, discharge summaries, and incident reports

• Graduate degree from CAPTE-accredited Physical Therapy program.
• Current state of MI license and CPR certification
• Ability to work efficiently/effectively in an autonomous environment
• Basic PC skills and knowledge, including but not limited to Electronic Medical Record (EMR) system, database software, and MS Office,
