Job summary

The Center For Physical Rehabilitation seeks a motivated PRN Physical Therapist to join our outpatient orthopedic clinics in the Grand Rapids area. Clinics operate Monday–Friday, 7 a.m.–7 p.m., offering flexible PRN opportunities to fit your schedule.

Ideal candidates hold a CAPTE-accredited PT degree, MI license, and CPR certification, with strong autonomous work skills and proficiency in EMR and MS Office. Join a supportive, patient-centered team committed to restoring life without limits.

Qualifications

• Graduate degree from CAPTE-accredited Physical Therapy program.
• Current MI license and CPR certification.
• Ability to work efficiently/effectively in an autonomous environment.
• Basic PC skills and knowledge, including EMR system and MS Office.

Responsibilities

• Assess patients to develop and initiate treatment plans based on evaluation results.
• Review the physician's referral and the patient's condition and medical records to determine the physical therapy treatment required.
• Provide direct patient care by established protocols and patient care guidelines.
• Document findings, progress, and instructions to patients and caregivers.
• Responsible for patient care performed by assistants and rehabilitation technicians.
• Complete a written plan of care, develop goals based on evaluation findings, and develop a comprehensive program to attain goals.
• Implement physical therapy treatment program and provide essential physical therapy treatments.
• Communicate effectively with patients/caregivers and professional colleagues, including physicians and insurance companies.
• Ensure timely documentation, including daily notes, weekly workers' compensation reports, progress reports, initial evaluations, discharge summaries, and incident reports.
