Job summary

LiveWell Homecare Agency, LLC is seeking a licensed physical therapist to evaluate patients, develop individualized treatment plans, and implement evidence-based interventions to improve outcomes. You will educate patients, discharge planning, and document care while coordinating scheduling and billing.

Collaboration with physicians, leadership, and a multidisciplinary team is essential. Flexibility to support across departments is required.

Qualifications

• Requires a valid license in the practicing state.
• Bachelor’s, Master’s, or Doctorate degree from an accredited program.
• BLS or Basic Cardiac Life Support certification.
• Strong communication, professionalism, and teamwork skills.

Responsibilities

• Provide physical therapy evaluations and develop individualized treatment plans.
• Implement evidence-based interventions to improve patient outcomes.
• Educate patients, perform discharge planning, and document care.
• Handle billing, scheduling, and supervise support staff.
• Collaborate with multidisciplinary team and communicate with physicians and leadership.
• May serve as clinical instructor and participate in department committees.
• Flexibly work across departments based on patient needs.
