Job summary

LiveWell Homecare Agency, LLC is seeking a dedicated Physical Therapist to provide high-quality home-based therapy in Virginia Beach and surrounding areas. You will assess and treat patients with mobility, strength, and pain management needs, and help families with exercises, self-care and safety education.

The role requires graduation from an accredited PT program, an active Virginia PT license, current CPR certification, and up to 2 years of experience, preferably in home healthcare.

Responsibilities

• Assessing and treating patients with mobility, strength, and pain management needs.
• Conducting assessments, documenting progress, and adjusting care plans accordingly.
• Guiding patients and families with exercises, self-care, and safety education.
• Collaborating with a home health team to meet individual care goals.
