Job summary
Key Rehab is seeking a Physical Therapist to join our team in a SNF setting in multiple capacities (Full time, Part time, PRN). You will evaluate patients, develop treatment plans, and collaborate with an interdisciplinary team to achieve patient goals.
The role includes supervising PTAs and balancing duties between Home Health and SNF settings, with potential weekend shifts. We welcome new graduates and experienced therapists, offering competitive pay, benefits, and opportunities for mentorship
Qualifications
- Graduation from an accredited Physical Therapist program.
- Active PT license in the state of practice.
- Adherence to Key Rehab’s Code of Ethical Conduct and applicable regulations.
Responsibilities
- Evaluate each patient and develop individualized treatment plans in collaboration with the interdisciplinary team.
- Supervise PTAs within the facilities as part of the action plan.
- Assist in coordinating care across Home Health and SNF settings, including a weekend schedule when applicable.
- The role supports flexible staffing in Full time, Part time or PRN capacities, aligning with patient needs.
