Total Care Therapy is seeking a compassionate Physical Therapist to join our growing team in Northwestern Ohio. You will evaluate, develop, and implement individualized treatment plans to improve mobility, reduce pain, and promote independence for residents in assisted and independent living communities.

Qualifikationen

• Active Physical Therapist license in the state
• CPR/BLS certification current
• Valid driver's license and reliable transportation

Aufgaben

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments including therapeutic exercise, gait training, balance training, transfer training, and mobility interventions.
• Reassess patient progress and modify treatment plans as needed.
• Supervise Physical Therapist Assistants (PTAs) per state regulations.
• Complete timely and accurate EMR documentation.
• Educate residents, caregivers and facility staff on plans, home programs, fall prevention, and safe mobility techniques.
• Collaborate with interdisciplinary team to deliver coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering high-quality therapy.
