Total Care Therapy is seeking a dedicated Occupational Therapist to join our Northeast Ohio team, focusing on residents in assisted living and independent living communities. You will evaluate, treat, and educate to maximize independence in ADLs, cognition, and safety, while collaborating with an interdisciplinary team.

Formación
- Current OT license in the state
- CPR/BLS certification current
- Valid driver's license with reliable transportation
- Experience in geriatrics/home health/assisted living or skilled nursing preferred but not required
Responsabilidades
- Evaluate residents and develop individualized OT plans of care.
- Provide OT interventions to improve ADLs, IADLs, UE function, cognition, balance, and safety.
- Assess and recommend adaptive equipment and environmental modifications.
- Reassess progress and modify treatment plans as needed.
- Supervise COTAs per state regulations.
- Document accurately in EMR.
- Educate residents, caregivers, and staff on home safety and energy conservation.
- Collaborate with interdisciplinary team for coordinated care.
