Evolve Therapy Services is hiring a Physical Therapist for an in-house Skilled Nursing Facility located near North Platte, Nebraska. The position offers full-time, part-time, or PRN options with flexible scheduling and a $10,000 sign-on bonus in addition to salary.

The role focuses on planning, directing, and providing physical therapy services, documentation, and compliant CMS reporting, to ensure high-quality resident care and collaboration with interdisciplinary teams.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents according to standards of practice and department procedures.
• Adhere to documentation and billing procedures for therapy services.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short and long-term goals; communicate findings to residents and care teams.
• Complete CMS-compliant documentation in NetHealth EMR systems.
