Job summary

Evolve Therapy Services, LLC seeks a dedicated Physical Therapist to join our in-house skilled nursing facility partner in South Sioux City, NE. You will provide comprehensive PT services, develop treatment plans, and communicate progress with residents and families.

The role requires a BS in Physical Therapy, an active state license, and commitment to ongoing professional development. A $10,000 sign-on/relocation bonus is offered along with full-time benefits.

Qualifications

• Must hold a Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Adhere to documentation and billing procedures.
• Develop and implement treatment plans with short and long-term goals.
• Communicate evaluation findings and treatment recommendations to residents, families, and interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR.
