Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility in North Platte, Nebraska. This full-time, part-time, or PRN position offers flexible scheduling and a $10,000 sign-on bonus along with a salaried compensation package.

Responsibilities include providing therapy services, documenting and billing per CMS standards, and collaborating with an interdisciplinary team to develop and implement treatment plans for residents.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsabilités

• Provide physical therapy services to residents in accordance with established standards of practice and department procedures.
• Adhere to documentation and billing procedures.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of resident therapy programs.
• Conduct screening of residents to determine need for intervention/treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short and long-term goals.
• Communicate evaluation findings and recommendations to residents, families, and the interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR systems.
