Job summary

Evolve Therapy Services, LLC is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility located in North Platte, NE. This full-time role offers a $10,000 sign-on bonus plus salaried compensation, with options for Part Time or PRN and flexible scheduling.

You will plan, organize, and direct Physical Therapy services in accordance with standards, regulations, and CMS expectations, while collaborating with an interdisciplinary team to ensure high-quality resident care.

Qualifications

• Bachelor degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Commitment to ongoing professional development.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Adhere to documentation and billing procedures.
• Demonstrate sound clinical judgment in plans and follow-up.
• Screen residents regularly to determine need for intervention.
• Screen referred residents to determine need for intervention.
• Evaluate residents to obtain data for treatment planning.
• Develop treatment plans with short and long-term goals.
• Communicate evaluation findings to residents, families, and team.
• Complete CMS-compliant documentation in NetHealth EMR.
• Perform other duties as assigned.
