Job summary

Laurel Park Rehabilitation And Healthcare Center in Elizabeth City, NC seeks a licensed Physical Therapist for full-time or part-time roles within a long-term care setting. The therapist will perform evaluations, develop and monitor treatment plans, and coordinate with the interdisciplinary team to optimize patient outcomes.

Responsibilities include documenting MD orders, progress notes, and discharge summaries, as well as participating in facility meetings and staff education as needed.

Qualifications

• Graduate of an accredited university with a B.S., Doctorate, MA or MS in Physical Therapy that is recognized by the APTA.
• Holds current license and/or registration in the state (as applicable)

Responsibilities

• Provide a comprehensive physical therapy evaluation based on MD’s orders.
• Document findings on the standardized evaluation form, accurately and adhering to guidelines.
• Provide a comprehensive treatment plan including long and short term goals, frequency, duration and treatment modalities.
• Ensure MD orders are obtained for evaluations, treatments, re-certifications and discharges.
• Provide comprehensive treatment to patients utilizing modalities and modify treatment as indicated while adhering to precautions.
• Document patient’s daily participation and progress on weekly progress notes.
• Document updated treatment plan in a timely manner as indicated.
• Document patient’s discharge and in-services on a discharge summary as indicated.
• Document any caregiver or discharge in-services on an in-service form as indicated.

Education

Physical Therapy degree (B.S./DPT/MA/MS) recognized by the APTA
