Qualifications

• BS Physical Therapy from an accredited program.
• Active MA Physical Therapy license OR candidates for licensure who have submitted a complete and accepted application for licensure.
• American Heart Association – Basic Life Support (BLS) certification.

Responsibilities

• Examines patient and develops a detailed assessment and measurable treatment plan.
• Implements treatment and educates patient and family; monitors progress for discharge readiness.
• Initiates discharge planning and coordinates with multidisciplinary team.
• Maintains accurate medical records and documentation per policy.
• Meets department productivity standards and participates in quality improvement activities.
