Qualifications

• BS Physical Therapy from an accredited program.
• Active MA Physical Therapy license; BLS certified.
• Minimum two years licensed physical therapist experience.
• Knowledge of physical therapy theory and practice.
• Strong communication and documentation abilities.

Responsibilities

• Assess, plan, and participate in rehabilitative programs to improve physical function.
• Develop treatment plans and outcomes with input from patients and care team.
• Coordinate discharge planning and multidisciplinary care.
• Maintain accurate documentation of evaluations, progress notes, and discharge summaries.
• Meet productivity standards and mentor staff.
• Provide training and education to staff and supervise students.
• Lead quality improvement and program development.
