Job summary

Reliant Rehabilitation is seeking a qualified Physical Therapist to deliver high-quality therapy services across care communities. You will screen, evaluate, and treat patients, develop and implement plans, and coordinate discharge planning while ensuring regulatory compliance.

Expect to supervise PTAs and rehab technicians, manage cases, and maintain professional standards. APTA-accredited education and current state licensure are required.

Qualifications

• Graduate of an APTA-accredited Physical Therapy program.
• Valid state license to practice as a Physical Therapist.

Responsibilities

• Screen and evaluate patients to determine therapy needs.
• Develop and implement individualized treatment plans.
• Discharge planning and transition of care.
• Supervise Physical Therapist Assistants (PTAs) and Rehab Technicians.
• Manage cases and coordinate care with facilities and clients.
• Uphold professional conduct and ensure compliance with state and federal regulations.
