Job summary

Lincoln Meadows Care Center in Lincoln, CA is seeking a Physical Therapist (PT) to join our rehabilitation department and help residents regain independence and improve quality of life. This full-time role offers a competitive hourly rate and opportunities to grow within a supportive environment.

You will evaluate patients, develop individualized treatment plans, document progress, and collaborate with an interdisciplinary team.

Qualifications

Active CA Physical Therapist License or eligibility.
Graduate of an accredited Physical Therapy program.
Familiarity with EHR systems such as Net Health or PointClickCare is a plus.
Team player with strong communication skills.
Passionate about making a positive impact in residents' lives.
SNF experience not required.

Responsibilities

Evaluating, creating individualized treatment plans and celebrating patient progress.
Maintain accurate and timely documentation in accordance with facility guidelines and procedures.
Helping residents regain strength, mobility, confidence, and independence.
Working alongside a supportive interdisciplinary team that truly works together.
