Job summary

Lincoln Meadows Care Center in Lincoln, CA is seeking a full-time Physical Therapist to join our rehabilitation department and help residents regain independence with individualized care.

You’ll evaluate patients, create treatment plans, document progress, and collaborate with an interdisciplinary team in a supportive, welcoming setting.

Qualifications

• Active CA Physical Therapist license or eligible.
• Graduate of an accredited Physical Therapy program.
• Familiarity with EHR systems is a plus.
• Strong communication and teamwork skills.
