Job summary

Lincoln Meadows Care Center in Lincoln, CA is seeking a Physical Therapist (PT) for full‑time, part‑time, or per‑diem roles. The position offers competitive pay ($55–$65/hour DOE) and a supportive rehab team dedicated to patient‑centered care.

As part of the rehabilitation department, you will evaluate residents, develop individualized treatment plans, document accurately, and help patients regain strength and independence while collaborating with an interdisciplinary team and engaging families.

Qualifications

• Active CA Physical Therapist License or eligibility.
• Graduate of an accredited Physical Therapy program.
• Familiarity with EHR systems such as Net Health or PointClickCare is a plus.
• Team player with strong communication skills.
• New grads welcome!

Responsibilities

• Evaluating, creating individualized treatment plans and celebrating patient progress.
• Maintain accurate and timely documentation in accordance with facility guidelines and procedures.
