Linwood Meadows Care Center in Visalia, CA is seeking a compassionate Physical Therapist for per diem coverage. The PT will evaluate residents, develop personalized treatment plans, and help regain mobility, strength, and independence.

Eligible candidates hold a CA PT license and have experience with Casamba and PointClickCare. The role offers per diem scheduling with competitive hourly pay and opportunities for ongoing training.

Qualifications

• Doctorate or Master's Degree in Physical Therapy from an accredited program required.
• Current California Physical Therapist license in good standing.
• Experience with Casamba and PointClickCare preferred.

Responsibilities

• Evaluate residents and determine physical therapy needs and rehabilitation potential.
• Develop, implement, and modify individualized treatment plans.
• Provide treatment focused on gait training, transfers, balance, strengthening, mobility, and pain management.
• Communicate with physicians, nursing staff, residents, and families regarding progress and goals.
• Participate in care plan meetings and interdisciplinary team conferences.
• Complete timely, accurate, and compliant documentation.
• Educate residents and caregivers on exercises, mobility safety, and fall prevention.
