Job summary

Concept Rehab, Inc. seeks a dedicated Physical Therapist to evaluate and treat patients with mobility and function challenges in our Ohio facility. You will collaborate with physicians, families, and staff, ensure accurate documentation, and promote professional, compassionate care daily.

The role requires DPT/BA or higher, state licensure, and a commitment to ongoing education and team leadership in a supportive, therapist‑owned environment.

Qualifications

• Licensed in the state(s) where services are provided as a Physical Therapist.
• Doctor of Physical Therapy (DPT) or equivalent advanced degree.
• Strong documentation and communication skills.

Responsibilities

• Evaluate patients for rehabilitation within 24–48 hours of orders.
• Develop and implement effective treatment plans.
• Treat patients according to plan of care.
• Supervise the Licensed Physical Therapy Assistant and collaborate with the team.
• Communicate progress with physicians, managers and staff.
• Participate in Plan of Care and Interdisciplinary Team Meetings.
• Meet or exceed productivity standards.
• Lead in‑service trainings and discharge planning as needed.
• Record notes, discharge summaries and daily billing.
• Educate families and facility staff on maintenance programs.
• Occasionally work beyond usual hours for emergencies or holidays.
• Demonstrate Concepts Rehab values and professional image.
