Job summary

Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility in North Platte, NE. This full-time role includes a $10,000 relocation or sign-on bonus plus salaried compensation. Flexible scheduling is also available for part-time or PRN positions.

The PT will plan, organize, and direct therapy services, perform evaluations, develop treatment plans, and ensure CMS-compliant documentation using NetHealth EMR. Active licensure and ongoing CEU are required.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide PT services to residents in accordance with standards.
• Document and bill PT services per procedures.
• Evaluate and develop treatment plans with goals.
• Communicate findings to residents, families, and interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR.
