Job summary

Evolve Therapy Services, LLC is seeking a licensed Physical Therapist for an in-house Skilled Nursing Facility in North Platte, NE. The role offers full-time, part-time, or PRN options with flexible scheduling and a $10,000 relocation or sign-on bonus plus salaried compensation.

Responsibilities include evaluating residents, developing treatment plans, documenting in NetHealth EMR, and collaborating with the interdisciplinary team to ensure high-quality resident care.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents per established standards and procedures.
• Adhere to documentation and billing procedures for therapy services.
• Develop and implement individualized treatment plans with short and long-term goals.
• Screen residents to determine need for intervention and treatment.
• Evaluate residents to collect data for planning and implementation of therapy.
• Communicate findings and recommendations to residents, families, and interdisciplinary team.
• Complete compliant, medically necessary CMS documentation in NetHealth EMR.
• Perform additional duties as required to achieve quality resident care.
