Job summary

Evolve Therapy Services, LLC is hiring a Physical Therapist for an in-house Skilled Nursing Facility in North Platte, NE. The role offers full-time, part-time, or PRN options with flexible scheduling and a $10,000 sign-on bonus, and requires an active PT license.

You will plan, direct, and provide PT services, with emphasis on assessment, treatment planning, and documentation in NetHealth EMR, in collaboration with an interdisciplinary team to ensure resident quality care.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and company procedures.
• Adhere to documentation and billing procedures.
• Demonstrate sound judgment in evaluation, planning, implementation and follow-up of resident therapy programs.
• Conduct regular screenings to determine need for intervention or treatment.
• Develop treatment plans with short and long-term goals and methods to achieve them.
• Communicate evaluation findings and treatment plans to residents, families, and the interdisciplinary team.
• Ensure compliant and medically necessary documentation in NetHealth EMR systems.
