Qualifications

• Current state licensure or eligibility to obtain licensure in Minnesota, or license in another state if eligible for transfer.

Responsibilities

• Provides and directs physical therapy services to patients, including assessment, treatment, program planning and implementation, and related documentation.
• Conducts screenings to determine need for intervention and treatment.
• Develops treatment plans with short- and long-term goals and methods to achieve them.
• Supervises and directs PT Aides and rehab staff as required.
• Documents results of assessments and treatment and participates in discharge planning.
