Job summary

Northwoods Rehab in Minnesota is seeking a Physical Therapist to provide and direct therapy services to patients, under physician orders, and to develop treatment plans aligned with patient goals.

You will supervise staff, coordinate with the care team, document outcomes, and participate in discharge planning while maintaining compliance with Northwoods Rehab policies and infection control standards.

Qualifications

• Bachelor (or higher degree) of Science in Physical Therapy from an accredited program.
• Successful completion of National Certification Examination for Registered Physical Therapist.
• Current state licensure or license eligible.
• Assumes responsibility for ongoing continuing education and professional development.

Responsibilities

• Provides and directs physical therapy services to patients, including the assessment, treatment, program planning and implementation, related documentation and communication. Functions under physician’s orders.
• Evaluates patients to obtain data necessary for treatment planning and implementation and communicates evaluation findings to patients, family and other team members.
• Establishes individual patient schedules in conjunction with other team members and develops treatment plans with short and long-term goals.
• Coordinates treatment plan with patient, family and other team members; implements and monitors response to intervention.
• Supervises and directs Licensed Physical Therapy Assistant, Physical Therapy Aides and students; participates in discharge planning.
