Responsibilities

• Provides and directs physical therapy services to patients under physician orders.
• develops treatment plans with short and long‑term goals and coordinates with the rehab team.
• Documents results of assessments, treatments and termination of services.
• Supervises Licensed Physical Therapy Assistant and aides as required.
• Ensures compliance with infection control and safety standards.
• Travels to perform home assessments within reasonable distance if needed.
