Job summary

Northwoods Rehab in Minnesota is seeking a licensed Physical Therapist to provide and direct therapy services under physician orders. You will assess, plan, implement and document care while coordinating with patients, families and the care team.

You will supervise PT aides and assistants, participate in discharge planning, and maintain compliant documentation and safety standards throughout the day.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Successful completion of National Certification Examination for Registered Physical Therapist.
• Current state licensure or license eligible.
• Assumes responsibility for ongoing continuing education and professional development.
• Physical requirements: medium work.

Responsibilities

• Provides and directs physical therapy services to patients, including assessment, treatment, planning and documentation under physician orders.
• Initiates referrals when appropriate and in accordance with Northwoods Rehab policies.
• Conducts screenings of patients to determine intervention needs.
• Evaluates patients to obtain data for treatment planning and implementation.
• Develops treatment plans with short and long-term goals and coordinates with the team.
• Implements treatment as outlined and monitors response to achieve goals.
• Supervises PT assistants and aides and participates in discharge planning.
• Maintains documentation and adheres to safety and infection control standards.
