Job summary

NeoPath Health, Inc., a Minnesota based administrator of employer based medical services, is seeking a full-time physical therapist to work at an employer-based clinic in the Green Bay, WI area.

The Physical Therapist collaborates with a clinical team to promote mobility, function, independence, and quality of life, performing evaluations, developing plans of care, and delivering timely, evidence-based therapy within scope of practice, with a patient-centered approach.

Qualifications

• Doctor of Physical Therapy (DPT) degree or CAPTE-accredited program meeting licensure requirements.
• Current licensure as a Physical Therapist in good standing.
• Three or more years of clinical physical therapy experience preferred.
• Strong organizational skills and the ability to manage multiple priorities.
• Proficiency with computer tools: Excel, Word, Outlook; EHR experience preferred.

Responsibilities

• Perform comprehensive physical therapy evaluations and assess movement and functional limitations.
• Develop individualized plans of care with measurable goals based on evaluations.
• Provide therapeutic exercise, neuromuscular re-education, gait and balance training, and manual therapy as appropriate.
• Educate patients and caregivers on diagnosis, treatment plans, home exercise programs, and safety.
• Monitor response to treatment and modify plans to achieve goals.
• Maintain accurate documentation in the electronic health record.
