Job summary

Qualifications

• Graduate of an accredited Physical Therapy program.
• Active Nebraska license or a compact-state license.
• Basic Life Support (BLS) certification required.

Responsibilities

• Evaluate residents and create individualized plans of care to improve mobility and independence.
• Provide evidence-based interventions to improve mobility, strength, balance, endurance, pain management, and functional independence.
• Educate residents and families on treatment plans, home exercise programs, safety strategies, and ongoing care recommendations.
• Document, code, and maintain timely clinical records.
• Collaborate with physicians, nursing, therapy team members, and other interdisciplinary partners to coordinate resident care.
• Assist with discharge planning, transitions between care settings, and supervision of therapy staff as appropriate.
