NeoPath Health, Inc., a Minnesota-based administrator of employer-based medical services, is seeking a part-time physical therapist to work at an employer-based clinic in Shawano, Wisconsin.

The Physical Therapist evaluates movement and functional limits, develops individualized care plans, and provides evidence-based PT services while collaborating with a clinical team to deliver an exceptional patient care experience.

Qualifications

• DPT degree with licensure in good standing.
• CAPTE-accredited program graduate or state licensure.
• Current PT license in good standing.
• 3+ years of clinical PT experience preferred.
• Strong organizational skills and ability to manage multiple priorities.
• Computer skills including Excel, Word, Outlook; EHR experience preferred.

Responsabilités

• Perform comprehensive PT evaluations and develop goals.
• Provide therapeutic exercise, neuromuscular re-education, gait and balance training.
• Use modalities to reduce pain and improve function.
• Educate patients and caregivers on treatment plans and home programs.
• Monitor response and adjust plan of care as needed.
• Document evaluations, sessions, progress, and discharge planning in EHR.
• Collaborate with care team to coordinate patient-centered care.
• Recommend assistive devices and home/work modifications.
• Perform other duties as assigned.
