Job summary

NeoPath Health, Inc. seeks a full-time Physical Therapist to join an employer-based clinic serving the Green Bay area. The PT will evaluate patients, develop individualized care plans, and deliver evidence-based therapy to promote mobility and independence.

The role requires a DPT or equivalent, current licensure, CAPTE accreditation or equivalent, and 3+ years of clinical PT experience. Excellent organizational skills and proficiency with Excel, Word, Outlook, and EHR are preferred.

Qualifications

• DPT or eligible PT degree with licensure per state requirements.
• CAPTE-accredited program graduation or equivalent licensure.
• Current PT license in good standing.
• Three+ years of clinical PT experience preferred.
• Strong organizational and multi-tasking abilities.
• Proficient computer skills; Excel, Word, Outlook; EHR experience preferred.

Responsibilities

• Evaluate movement and functional limitations and develop plans of care.
• Provide therapeutic exercise, manual therapy, and gait/balance training.
• Educate patients and caregivers on diagnosis, home programs and safety.
• Monitor progress and modify plans to achieve goals.
• Document evaluations and treatment sessions accurately in the EHR.
• Collaborate with the clinical team to coordinate care and referrals.
• Identify needs for assistive devices or home/workplace modifications.
