Job summary

Lakes Region Community Services is seeking a part-time Physical Therapist (PT) to join our Family Resource Center team in Laconia, NH. The role includes assessing development levels and eligibility for Early Intervention services as part of a trans-disciplinary team.

The PT will develop and implement an Individualized Family Service Plan with families and other appropriate individuals; there is flexibility to move toward full-time.

Qualifications

• DPT required.
• NH PT license required.
• 1–2 years of experience with young children and families preferred.
• Valid driver's license, reliable vehicle, and automobile insurance required.

Responsibilities

This is a part-time opportunity with flexibility to become a full-time position. The position includes assessing as part of a trans-disciplinary team to determine current development level and eligibility for Early Intervention services. Additionally, the PT will develop and implement an Individualized Family Service Plan with family and other appropriate individuals.

Responsibilities

The right candidate will have experience working with young children and their families in the community, be comfortable with family-centered approaches, be committed to working in a team environment, and be flexible with scheduling. Experience with and knowledge of Early Supports and Services is preferred.

Requirements

• The qualified candidate is required to possess a minimum of a Doctor of Physical Therapy (DPT).
• Additionally, a license from the NH Office of Professional Licensure and Certification is required.
• 1-2 years of professional experience and working with young children and families is preferred.
• A valid driver's license, reliable vehicle, and automobile insurance are required.
