Job summary

Lakes Region Community Services is seeking a part-time Physical Therapist (PT) to join our Family Resource Center team in Laconia, NH. The role includes assessing development levels and eligibility for Early Intervention services as part of a trans-disciplinary team.

The PT will develop and implement an Individualized Family Service Plan with families and other appropriate individuals; there is flexibility to move toward full-time.

Qualifications

• DPT required.
• NH PT license required.
• 1–2 years of experience with young children and families preferred.
• Valid driver's license, reliable vehicle, and automobile insurance required.

Responsibilities

• Assess development levels and eligibility for Early Intervention services as part of a trans-disciplinary team.
• Develop and implement an Individualized Family Service Plan with the family and other appropriate individuals.
• Collaborate with Family Resource Center team to support young children and families.
• Flexibility with scheduling; opportunity to move toward full-time.
