Lake Sunapee Region VNA & Hospice is seeking a Homecare Physical Therapist to provide PT services to patients per physician plan of care. The PT initiates the program, instructs staff and family, and may supervise PT aides in accordance with policy and state regulations.
The role is full-time with flexible travel within service area; travel time and mileage reimbursement are provided, with a dynamic, multidisciplinary team in a culture that emphasizes teamwork and excellence.

As a Homecare Physical Therapist you will provide physical therapy services to patients according to a written physician's plan of care as defined in the state Physical Therapy Practice Act. The Physical Therapist (PT) initiates the physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences. The Physical Therapist (PT) supervises physical therapy assistants according to organization policy and state regulations.
