Job summary

Mount Carmel Health System in Central Ohio seeks a dedicated Physical Therapist to evaluate and treat patients referred for PT services. You will collaborate with care teams and maintain accurate records, ensuring quality care in a busy setting.

Responsibilities include planning, documenting, and communicating patient progress, with no weekends or holidays. Ohio licensure and BLS are required; Doctorate preferred. Relocation and growth opportunities available within Trinity Health.

Qualifications

• Graduate of an accredited physical therapy program.
• Current license to practice Physical Therapy in Ohio.
• Current BLS for Health Care Provider certification.
• One year of clinical experience preferred.
• Effective communication skills.
• Ability to travel to other sites as necessary.
• Ability to function with minimal supervision.
• Physically able to perform lifting and transferring.

Responsibilities

• Performs PT evaluation and establishes/implements treatment plans.
• Documents evaluation results, interventions, progress and discharge notes.
• Maintains proper billing for the services provided.
• Delegates appropriate procedures to PTA and reviews documentation.
• Collaborates with other health care providers on care planning.
