Job summary

Mount Carmel Health System, part of Trinity Health, is seeking a Physical Therapist to evaluate and treat patients referred for PT services. You will collaborate with caregivers to ensure quality care and maintain accurate records.

The role requires a Doctorate in Physical Therapy (or progress toward degree), current Ohio PT license, and BLS certification. Travel to other sites may be necessary; one year of experience is preferred.

Qualifications

• Graduate of an accredited physical therapy program; Doctorate degree preferred.
• Current license to practice Physical Therapy in the state of Ohio; BLS certification required.
• One year of clinical experience in the field preferred.
• Effective communication skills and good judgment in patient evaluation and treatment.

Responsibilities

• Performs PT evaluation and implements treatment plans for referred patients.
• Documents PT evaluation results, progress, and discharge notes.
• Maintains proper billing for PT services.
• Delegates procedures to the PTA and reviews/documentation.
• Communicates with health care providers regarding patient care and discharge planning.
