Trinity Health in Ludington, Michigan seeks a licensed Physical Therapist to provide patient-centered PT services. You will evaluate patient needs, develop and implement treatment plans, and document outcomes according to policy.

The role requires collaboration with an interdisciplinary team to achieve optimal functional recovery. Minimum qualifications include a Doctorate in Physical Therapy, current license or eligibility in Michigan, and BLS certification.

Qualifications

• Doctorate in Physical Therapy required (ACPA or equivalent)
• Bachelor’s in Physical Therapy required
• FSBPT certification or eligible to obtain licensure
• State of Michigan Physical Therapist license or eligible before hire
• American Heart Association BLS or equivalent

Responsibilities

• Provide treatment within scope of license and as required by law.
• Perform evaluation and clinical diagnosis of patient conditions.
• Set treatment goals with patient and family and document per standards.
• Collaborate with an interdisciplinary care team to implement PT plan of care.
