Job summary

Trinityhealth in Grand Haven, MI is seeking a licensed Physical Therapist for a full-time, day-shift outpatient clinic. The PT will assess and treat patients autonomously within the scope of practice and collaborate with a multidisciplinary team.

Responsibilities include evaluating patient needs, setting treatment goals with input from patients and families, and documenting in accordance with insurance and regulatory guidelines. Michigan licensure and a current BLS certification are required.

Qualifications

• Must hold a Doctorate in Physical Therapy and be eligible for Michigan licensure.
• FSBPT certification is required.
• Graduate license-eligible candidates considered.

Responsibilities

• Evaluate patient needs for physical therapy and provide treatment within licensure scope.
• Set treatment goals with input from patient and family, and document per standards.
• Collaborate with an interdisciplinary care team to implement PT plan.
