Job summary

Continuum Therapy Partners is seeking a PRN Physical Therapist for Elmwood Health & Rehab Center in West Carrollton, OH. You will evaluate and treat patients, coordinate with the care team, and educate patients and families on home exercises, while maintaining high standards of care.

The role offers daytime PRN scheduling (15–20 hours per week) and the opportunity to contribute to a supportive, therapist-led team without committing to a full-time schedule.

Qualifications

• DPT degree is required.
• State PT license is required.
• NPTE must be passed.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
