Job Description

As a Physical Therapist Case Manager, you will be responsible for managing patient care in a home health setting. Your primary duties will include assessing patient needs, developing treatment plans, and coordinating with other healthcare professionals to ensure comprehensive care.

Responsibilities

1. Conduct patient assessments and evaluations.
