Physical Therapist Case Manager: Home Health

Position Details

Location: Contra Costa County: Richmond (territory)

Type: Full-time

Responsibilities:

1. Perform evaluations in patients' homes and develop care plans.
2. Assist patients with mobility and independence.
3. Create detailed treatment plans.
4. Coordinate with physicians and report progress.

Qualifications

• Current unrestricted CA Physical Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge of local community resources and regulatory guidelines
• Experience in home health care and with high acuity patients preferred
