Job summary

TEEMA Solutions Group is seeking a licensed Physical Therapist (PT) to provide high-quality rehabilitation in a healthcare setting. The PT will evaluate patients, develop individualized treatment plans, implement interventions, and monitor progress to restore mobility and reduce pain.

The role requires active PT licensure, a DPT or equivalent, and the ability to collaborate with physicians and interdisciplinary teams. Prior experience in various rehab settings is preferred.

Qualifications

• Active Physical Therapist (PT) license in the state of practice.
• Current Basic Life Support (BLS/CPR) certification preferred or required based on facility policy.
• Doctor of Physical Therapy (DPT) degree or equivalent educational requirements as required by state regulations.
• Previous experience in acute care, outpatient, inpatient rehabilitation, skilled nursing, home health, or specialty-specific settings preferred.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on functional needs and physician referrals.
• Provide therapeutic interventions to improve mobility, strength, balance, flexibility, coordination, pain management, and functional independence.
• Monitor patient progress, modify treatment plans as appropriate, and document all evaluations, treatments, and outcomes accurately.
• Educate patients, families, and caregivers regarding treatment plans, home exercise programs, injury prevention, and discharge recommendations.
• Collaborate with physicians and interdisciplinary healthcare teams to coordinate patient care and achieve optimal rehabilitation outcomes.
• Maintain accurate and timely clinical documentation in accordance with facility policies and regulatory requirements.
• Ensure compliance with patient safety, infection prevention, and quality assurance standards.
• Treat patients across assigned care settings and age groups as required by the facility.
• Perform other job-related duties as assigned.
