Job summary

TEEMA Solutions Group is seeking a dedicated Physical Therapist (PT) to deliver patient-centered rehabilitation in a healthcare setting. You will evaluate patients, develop individualized treatment plans, and implement therapeutic interventions to restore mobility and function.

Collaborating with physicians, nurses, and interdisciplinary teams, you will monitor progress, educate patients and families, and ensure documentation complies with policy and regulatory standards.

Qualifications

• Active Physical Therapist (PT) license in the state of practice.
• Current Basic Life Support (BLS/CPR) certification preferred or required based on facility policy.
• Doctor of Physical Therapy (DPT) degree or equivalent educational requirements as required by state regulations.
• Previous experience in acute care, outpatient, inpatient rehabilitation, skilled nursing, home health, or specialty-specific settings preferred.
• Strong clinical assessment, communication, documentation, and patient education skills.
• Ability to work independently and collaboratively within an interdisciplinary healthcare team.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on functional needs and physician referrals.
• Provide therapeutic interventions to improve mobility, strength, balance, flexibility, coordination, pain management, and functional independence.
• Monitor patient progress, modify treatment plans as appropriate, and document all evaluations, treatments, and outcomes accurately.
• Educate patients, families, and caregivers regarding treatment plans, home exercise programs, injury prevention, and discharge recommendations.
• Collaborate with physicians and interdisciplinary healthcare teams to coordinate patient care and achieve optimal rehabilitation outcomes.
• Maintain accurate and timely clinical documentation in accordance with facility policies and regulatory requirements.
• Ensure compliance with patient safety, infection prevention, and quality assurance standards.
• Treat patients across assigned care settings and age groups as required by the facility.
• Perform other job-related duties as assigned.
