Job summary

TEEMA Solutions Group is seeking a compassionate Physical Therapist (PT) to deliver high-quality rehabilitation services. The PT will evaluate patients, design individualized treatment plans, and implement therapeutic interventions to restore mobility, reduce pain, and improve function in a healthcare setting.

The role requires an active PT license, a DPT or equivalent, and BLS/CPR certification as per policy.

Qualifications

• Active PT license in the state of practice.
• Current BLS/CPR certification as required by facility policy.
• Doctor of Physical Therapy (DPT) degree or equivalent as required by state regulations.
• Experience in acute care, outpatient, inpatient rehabilitation, skilled nursing, home health, or specialty settings preferred.
• Strong clinical assessment, communication, documentation, and patient education skills.
• Ability to work independently and collaboratively within an interdisciplinary healthcare team.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on functional needs and physician referrals.
• Provide therapeutic interventions to improve mobility, strength, balance, flexibility, coordination, pain management, and functional independence.
• Monitor patient progress, modify treatment plans as appropriate, and document evaluations, treatments, and outcomes.
• Educate patients, families, and caregivers regarding treatment plans, home exercise programs, injury prevention, and discharge recommendations.
• Collaborate with physicians and interdisciplinary healthcare teams to coordinate patient care and achieve optimal rehabilitation outcomes.
• Maintain accurate and timely clinical documentation in accordance with facility policies and regulatory requirements.
• Ensure compliance with patient safety, infection prevention, and quality assurance standards.
• Treat patients across assigned care settings and age groups as required by the facility.
• Perform other job-related duties as assigned.
