Job summary

Visiting Nurse Health System is seeking a Physical Therapist to provide compassionate home-based care in the Smyrna/Atlanta area. This role emphasizes clinical skill, empathy, and a comforting presence for patients in their homes.

You will conduct initial assessments, create and adjust treatment and discharge plans, and collaborate with an interdisciplinary team to ensure high-quality, compliant care while driving between patient homes.

Qualifications

• Doctorate from an accredited Physical Therapy program.
• Current GA Physical Therapy license.
• CPR certification and valid driver's license required.
• Reliable transportation and automobile insurance.
• Experience with Medicare/Medicaid regulations and compliance in home health.
• Strong documentation and assessment skills (e.g., OASIS, care plans, discharge planning).
• Ability to educate patients, families, and interdisciplinary team members.

Responsibilities

• Perform initial physical therapy assessments for home health patients.
• Develop and revise individualized treatment plans and discharge plans.
• Coordinate with interdisciplinary team members to create individualized care plans.
• Deliver compassionate, skilled therapy in the home setting.
• Educate patients, families, and caregivers on care techniques and recovery.
• Document patient progress and maintain records in compliance with regulations.
• Supervise PTAs and HHAs, including supervisory visits.
• Drive to patient homes and work independently in the field.

What You’ll Do:

• Supervise PTAs and HHAs, including conducting supervisory visits
• Comply with Medicare, Medicaid, and insurance regulations
• Drive to patient homes and work independently in the field
• Provide emotional support to patients and loved ones during challenging times
