Qualifications

• Graduated from an accredited nursing program (Diploma/Associate/BSN preferred).
• Current Georgia RN license and valid CPR certification.
• Valid driver’s license, reliable transportation.
• Home health experience preferred.
• Knowledge of Medicare/Medicaid regulations related to hospice/home health care.
• Strong clinical nursing skills and ability to work independently in home settings.

Responsibilities

• Develop and implement individualized care plans with physician orders and interdisciplinary teams.
• Modify care plans as patient conditions change and evaluate outcomes.
• Educate patients and families on disease processes, self-management, and prevention of hospitalizations.
• Complete and submit OASIS documents and visit notes within VNHS guidelines.
• Communicate patient status changes clearly with physicians and team members.
• Supervise LPNs and HHAs per VNHS policy.
• Participate in SOC/ROC calls and weekly team conferences.
• Deliver high-quality home-based care adhering to VNHS standards.
• Maintain competencies through in-services and continuing education.
• Schedule visits to optimize productivity and manage caseload.
• Precept newly hired nursing staff and coordinate with office staff.

Job Overview

Visiting Nurse Health System is hiring a Full-time/ Registered Nurse Case Manager for Hospice, responsible for coordinating care for a caseload of patients and ensuring it meets quality standards and utilization guidelines. The RN Case Manager assesses, plans, implements, monitors, and evaluates comprehensive care plans to ensure optimal patient outcomes. This position demands a compassionate, skilled nurse who thrives in a home-based environment and values patient education and family engagement.

What You’ll Do:

• Develop and implement individualized care plans in accordance with physician orders by assessing patient needs and collaborating with the interdisciplinary team.
• Modify and update care plans as patient conditions change, regularly evaluating their effectiveness.
• Provide patient and family education on disease processes, self-management, and strategies to prevent complications such as hospitalizations.
• Complete and submit OASIS documents, visit notes, and other required records within VNHS timeliness guidelines.
• Communicate changes in patient status clearly with patients, physicians, and team members, documenting appropriately.
• Offer emotional support and connect patients/families with MSW and other community resources as needed.
• Supervise LPNs and HHAs per VNHS policy, ensuring care plans are in place and meet patient needs.
• Participate in SOC/ROC conference calls for new or resuming patients and in weekly team conferences.
• Deliver competent, high-quality care in the home, adhering to VNHS policies and nursing standards.
• Maintain required competencies through in-services and continuing education.
• Schedule patient visits efficiently in accordance with orders and manage case load to optimize productivity.
• Precept newly hired nursing staff and communicate effectively with the team and office staff regarding patient status or concerns.

• Graduated from an accredited nursing program (Diploma, Associate, or BSN preferred).
• Hold current licensure by the Georgia State Board of Nursing and valid CPR certification.
• Possess a valid driver’s license, reliable transportation, and current auto insurance.
• Have prior home health experience if applying as PRN staff.
• Demonstrate knowledge of Medicare, Medicaid, and funding source regulations as applied to hospice/home health care.
• Exhibit advanced clinical nursing skills and comfort collaborating with interdisciplinary teams.
• Have proven decision-making, time management, and interpersonal skills, functioning independently in a home environment.
• Are proficient with computers and basic technology for documentation, email, HR systems, and online training.
